POLICE INFORMATION CHECK — CALGARY POLICE SERVICE
Duty of Care
RHC recognizes the immense value of the many
volunteers who support and participate in RHC and its goals. RHC also has an
obligation to take
measures to care for and protect our membership
(underage hockey players).Starting in the 2009-2010 season, to help provide a safe and
secure
environment and to provide assurance to all those
involved in RHC, we have adopted a mandatory City of Calgary Police Service (CPS)
screening process for all coaches and assistant coaches volunteers.
To All Coaches and Assistant Coaches
Fill out form PRIOR to Coaches meeting and
BRING 1)
2 pieces of original ID (from list below)
2) photocopy of the above ID (there is NO photocopier
available at meeting)
LIST OF ACCEPTABLE IDENTIFICATION
(one from each section)
1) 1 piece of photo
id (1-driver's license, 2-motor vehicles registry photo ID, 3-native status
card, 4-passport,5- citizenship card,
6-photo ID from a registry office, 7-firearms
P.A.L., 8-military ID card)
2) 1 other
government issued id (9-birth certificate, 10-provincial health care card, 11-social
insurance card, 12-immigration documents, 13- marriage certificate, 14-permanent
residency card)
3) NOT
ACCEPTABLE: Student ID; Credit Cards

BRING THIS COMPLETED 2 PAGE FORM WITH YOU TO
THE COACHES MEETING.
RHC has signed
a Memorandum of Understanding with the CPS in order to provide this police
check for RHC coaches. DO NOT GO TO
THE CPS
CLEARANCE OFFICE WITH YOUR FORM as this would violate the
agreement we have with CPS. The cost of screening is borne by RHC. Your CPS
clearance is good for 3 years.
Once all forms are completed, RHC will send them into
the CPS — no copies will be kept by RHC.
The CPS in turn will complete a search based
provided
and will return to the RHC Registrar a form indicating either: (1) no criminal
record is indicated based on the information provided;may
exist for the individual based on the information provided. No other
information will be disclosed to RHC at that time and only the Registrar
from
RHC will see your form and the response letter from the CPS. All correspondence
will be treated with strict confidentiality.
The President will either receive
a "No Issues" clearance or a "Not Clear" report from the Registrar. Any report
of a "Not Clear" will result in the
volunteer not coaching in the RHC.If the
volunteer wishes to challenge or follow up on their finding, it will be their
responsibility to contact the
CPS(403) 206-2052 and arrange an appointment. The
applicant will be required to submit to fingerprinting to prove that they are
not subject of
a conviction issued by CPS if he/she
wishes to still be considered as a volunteer. This process and all
communication will be and shall remain strictly
confidential.
